Job Summary

St. Mary's Home Care and Hospice is seeking a compassionate and skilled Registered Nurse to provide high-quality care to patients in their homes. As a Home Health Care RN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities

* Provide one-to-one care to patients in their homes, focusing on their unique needs and goals
* Collaborate with our interdisciplinary team to develop and implement personalized care plans
* Utilize advanced technology and clinical knowledge to deliver exceptional care and outcomes
* Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination

Requirements

* Graduate of an approved nursing education program
* Licensure as a Registered Nurse in the State of Georgia
* One (1) year experience as a professional acute care nurse
* Home care experience preferred but not required
