Palliative Care Nurse Practitioner Opportunity

We are seeking a skilled Palliative Care Nurse Practitioner to join our team at HighPoint Palliative Care in Gallatin, TN, a part of LHC Group. Responsibilities- Provide direct patient and family care through palliative care consultation to ensure specialty services to patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.
- Develop and implement effective plans of care based on physical, emotional, spiritual, psycho/social, ethical, educational, safety, and related criteria.
- Collaborate with interdisciplinary teams to provide comprehensive care and support to patients and families.
- Participate in ongoing education and professional development to stay current with best practices in palliative care.
- RN/APRN licensure in the state of practice.
- Nurse Practitioner/CNS with prescriptive authority and a minimum of 5 years of clinical nursing experience, with at least 1 year as an APRN in Hospice, Palliative Care, or Oncology.
- Advanced practice palliative care certification (ACHPN) is preferred, but not required.
