We are seeking a skilled Care Manager RN to join our team at Providence Clinical Network. As a key member of our care team, you will work closely with patients, families, and healthcare professionals to deliver high-quality, patient-centered care.
About the JobThe RN Case Manager will be responsible for coordinating care for an assigned panel of patients, collaborating with a multidisciplinary team to ensure comprehensive and coordinated care. You will work in a dynamic environment where no two days are the same, and your skills and expertise will be valued and respected.
Key Responsibilities- Coordinate patient care with a focus on quality, safety, and efficiency
- Collaborate with healthcare teams to develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care
- Monitor patient outcomes and adjust care plans as needed
To be successful in this role, you will need:
- A degree from an accredited school of nursing
- An Alaska Registered Nurse License upon hire
- At least 2 years of professional nursing experience
