Job Description

As a Caregiving Professional with Harbor Health Services, you will be an integral part of our team, working collaboratively to provide high-quality patient care. Your responsibilities will include:

  • Care Planning and Coordination: Develop and implement individualized care plans that cater to the unique needs of each patient.
  • Patient Education: Educate patients and their families on various aspects of healthcare, promoting self-management and empowerment.
Requirements

To succeed in this role, you should possess:

  • Education: A degree in Nursing or related field.
  • Experience: Minimum 1-2 years of experience in patient care settings.
  • Skills: Excellent communication, interpersonal, and problem-solving skills.
