Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Specialist. In this role, you will provide follow-up visits for home health patients, ensuring they receive the best possible care and support.

About the Role:

  • Provide skilled nursing visits for home health patients, assessing their medical needs and developing personalized care plans.
  • Communicate effectively with patients' physicians, reporting on their condition, progress, and any concerns.
  • Prepare accurate and comprehensive reports, documenting patient records and updates.
  • Assist with medication management, including dispensing prescribed medications and monitoring patient adherence.
  • Monitor vital signs and record them as required.
  • Maintain accurate medication inventories, ordering supplies as needed.
  • Ensure infection control practices are followed at all times.
  • Report any hazardous conditions or equipment to your supervisor promptly.
  • Keep your supervisor informed of supply needs and maintain confidentiality of patient information.
  • Represent our organization professionally at all times, interacting with visitors, staff, and residents.
  • Avoid absences from work, adhering to scheduled shifts.
  • Participate in wound care initiatives and protocols.

Requirements:

  • High school diploma or equivalent.
  • Current RN license in good standing in the state where you practice.
  • CPR/First Aid certification.
  • Up-to-date TB test results.

Compensation: The estimated annual salary for this position is $ $85,000, commensurate with experience and qualifications. We offer a competitive benefits package, including comprehensive health insurance, paid time off, and opportunities for professional growth and development.
