Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Field RN Case Manager in Tulsa, OK. As a Field RN Case Manager, you will be responsible for assessing the health status of members, developing and implementing individualized care plans, and coordinating with interdisciplinary teams to ensure seamless care delivery.

Responsibilities

  • Assess the health status of members and develop personalized care plans to address their unique needs
  • Collaborate with interdisciplinary teams to coordinate care delivery and ensure continuity of care
  • Provide education and counseling to patients and families on disease management, condition trajectory, and follow-up recommendations
  • Document patient information accurately and thoroughly in electronic medical records
  • Maintain credentials essential for practice, including licensure, certification, and continuing education units
  • Demonstrate commitment to Optum's mission, core values, and goals, and integrate values of compassion, integrity, performance, innovation, and relationships into daily practice

Requirements

  • Current unrestricted licensure as an RN in the state of Oklahoma
  • 2+ years of experience as an RN, preferably in home health or geriatric care
  • Experience in assessing the medical needs of patients with complex behavioral, social, and/or functional needs
  • Computer literacy and ability to navigate electronic medical records
  • Proven ability to work effectively with diverse care teams in various settings, including non-clinical settings (primarily patient homes)
  • Ability to travel up to 75% of the time for field-based work, with a valid driver's license

Preferred Qualifications

  • Home health experience
  • Geriatric experience
  • Proven computer skills, including use of electronic medical records
  • Effective time management and communication skills
  • Excellent administrative and organizational skills, with the ability to effectively communicate with seniors and their families

Compensation

The estimated annual salary for this position is $80,000 - $100,000, depending on experience and qualifications.
