Job Summary:

The Care Specialist will play a vital role in facilitating patient education efforts and applying various methodologies to support individuals across the continuum of care. This position requires specialty-specific clinical knowledge and the ability to provide individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers.

Key Responsibilities:

  • Collaborate with healthcare team members to create a high-quality, efficient, safe, and exceptional caregiver and patient experience.
  • Develop and implement educational programs, including in-person and virtual classes, events, and materials.
  • Conduct technical and professional analyses of educational programs and make recommendations for improvement.
  • Provide continuous training and education to caregivers, patients, employees, and the community.
  • Serve as a consultant to other departments within Henry Ford Health System.

Requirements:

  • Bachelor's degree in Nursing or Master's in Social Work, required.
  • Minimum 3 years hospital and/or community health experience, required.
  • Strong computer skills and knowledge of Windows, Outlook, and Microsoft Office.
  • Registered Nurse (RN) with a valid, unrestricted State of Michigan license or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
