As a Care Coordinator RN, you will play a vital role in overseeing the progression of care and discharge planning for patients requiring these services. Your expertise will be essential in promoting quality of care, cost-effective outcomes, and adherence to hospital policies, standards of practice, and Federal and State regulations.

Key Responsibilities
  1. Conduct discharge planning assessments and develop multidisciplinary discharge plans that engage patients, families, and healthcare teams.
  2. Collaborate with utilization management, nursing, physicians, and ancillary departments to ensure timely and smooth transitions to post-acute services.
  3. Identify and mitigate risks for readmission, implementing interventions to ensure optimal patient outcomes.
  4. Deliver patient notifications and related documentation, while providing patient education and advocacy.
  5. Participate in performance improvement teams and programs, adhering to hospital policies and standards of practice.
Qualifications

To be successful in this role, you will need:

  • Graduate of an accredited school of nursing.
  • Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.

Preferred qualifications include:

  • Bachelor's Degree in Nursing (BSN) or related healthcare field.
  • At least five years of nursing experience.

Required licensure and certifications include:

  • Current licensure as a Registered Nurse in the state of Washington (RN-WA).
  • BLS required within 3 months of hiring if located within hospital.

Preferred certifications include Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification.
