Job Description

We are seeking a skilled Inpatient Social Worker to join our team at LifeBridge Health. As a Patient Care Advocate, you will play a critical role in promoting patient and family wellness, improving care outcomes, and ensuring access to appropriate hospital and community resources.

The ideal candidate will have a Master's degree in Social Work (MSW) and 3-5 years of experience in social work. A Licensed Master's Social Worker (LMSW) is required, with a Licensed Clinical Social Worker (LCSW/LCSW-C) being preferred.

You will collaborate closely with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Your expertise will help us deliver exceptional patient care and improve health outcomes in our community.
