Job Summary

We are seeking a dedicated Registered Nurse to join our Telemetry Unit team. As a Nurse Care Coordinator, you will be responsible for delivering exceptional patient care, collaborating with interdisciplinary teams, and promoting patient education and engagement.

Responsibilities
  • Patient Assessment and Care Planning: Conduct thorough assessments, develop individualized care plans, and implement evidence-based interventions to enhance patient outcomes.
  • Interdisciplinary Collaboration: Work closely with physicians, nurses, therapists, and other healthcare professionals to ensure seamless care coordination and communication.
  • Patient Education and Support: Provide patients and families with clear instructions, resources, and emotional support to empower them in their healthcare journey.
  • Clinical Decision Making: Utilize critical thinking and clinical judgment to make informed decisions regarding patient care, medication administration, and diagnostic testing.
  • Emergency Preparedness: Respond promptly and effectively to emergency situations, such as codes and rapid response team activations.
Requirements
  • Degree from an accredited school of nursing
  • Current licensure as a Registered Professional Nurse in the State of Illinois
  • BLS certification
  • Completion of Critical Care Course within 1 year of hire
  • ACLS certification within 6 months of hire
