About the Role

We are seeking a skilled Nursing Care Specialist to join our team at Norwalk Hospital Association.

Job Summary

The Nursing Care Specialist will provide direct patient care, collaborating with patients, family members, peers, physicians, and other healthcare professionals to deliver high-quality services. The successful candidate will assess patients' needs, develop care plans, implement medical and nursing regimens, and evaluate patient outcomes in accordance with hospital policies and standards.

Main Responsibilities
  1. Patient Assessment and Documentation: Conduct thorough patient assessments and reassessments as clinically indicated, documenting detailed information incorporating the plan of care, interventions, and outcomes reflecting patients' physical, mental, and emotional status.
  2. MEDICATION ADMINISTRATION: Follow the five Rights of Medication Administration, verbalizing medication information to patients/family members, assessing patients prior to administering medications, noting allergies, parameters, potential side effects, effectiveness, and intervening as needed. Reassess and document response according to hospital policy.
  3. Care Coordination and Supervision: Coordinate and supervise the delivery of nursing care, including delegating tasks, maintaining accountability for care delegated, and actively promoting a culture of safety.
  4. Quality and Safety: Actively work to promote a culture of safety by assessing patients for potential safety risks, adhering to hospital policies and procedures, initiating interventions related to identified risk factors, conducting bedside shift report and rounds, demonstrating quality by documenting the application of the nursing process, and ensuring patient hand-off when leaving for breaks.
  5. Patient Education: Assess learning needs of patients/family members, plan and provide appropriate instruction using applicable teaching methods, assess and document patient/family member response to teaching.
  6. Culture of Learning and Growth: Work to promote a culture of learning and growth by seeking experiences and formal/independent learning activities to maintain and develop clinical and professional skills/knowledge, teaching others to succeed through mentorship, and performing other duties as assigned within the state nursing scope of practice.
Requirements and Qualifications
  • Education: Associate's degree in Nursing
  • Licensure: Current and unobstructed Connecticut Registered Nurse License (CT Hospitals)
  • Certification: BLS, ACLS; American Nurses Association endorsed Certifications or Academy of Medical-Surgical Nurses Certification Preferred
  • Working Conditions: Manual: significant manual skills/motor coordination & finger dexterity; Occupational: some occupational risk; Physical Effort: medium to heavy effort; May exert up to 35 lbs. force; Physical Environment: some exposure to dirt, odors, noise, human waste, etc.
