Geriatric Primary Care Specialist

As a Geriatric Primary Care Specialist, you'll play a crucial role in delivering high-quality patient care to older adults living in Senior Living Communities across Southern San Diego. Your expertise in geriatrics and ability to work with complex care patients will make a significant impact on our team.
We're looking for a skilled Nurse Practitioner who is passionate about providing person-centered care and working collaboratively with a multidisciplinary team. Key Responsibilities:
- Provide onsite primary care services to older adults, developing and executing detailed person-centered care plans to maximize patient outcomes
- Work closely with Medical Director to identify and develop policies addressing emerging disease(s) and issues affecting day-to-day practice, culture, or clinical guidelines
- Lead regular huddles and provide leadership to the clinical team, fostering a collaborative environment that promotes shared knowledge and best practices
- Collaborate with senior living management and care teams to implement Pine Park care plans and improve patient outcomes
About the Opportunity:
- Pine Park Health offers a competitive salary range of $135,000 to $165,000 per year, plus equity and benefits tailored to your role and location

Requirements:
- Nurse Practitioner with geriatrics experience, preferably with a strong orientation towards collaboration and working closely with a multidisciplinary team
- Ability to work with tech-enabled care delivery tools, such as project management software, checklist management systems, remote monitoring tools, and more
- Comfortable with a fast-paced and changing environment, with a willingness to adapt to new challenges and opportunities
- Strong written and verbal communication skills, with a focus on clear, concise messaging that supports effective teamwork and patient care
- Flexible work schedule with travel within the San Diego area, with occasional driving between partner communities required
