About the RoleThis is an exciting opportunity to work as an independent contractor, providing one-on-one therapy services to patients in the comfort of their own homes. As a Home Health Physical Therapist with BAYADA, you'll have the freedom to build meaningful relationships with your clients, while also having the support of our experienced team behind you.
Responsibilities- Conduct thorough assessments and develop personalized care plans to address each patient's unique needs and goals.
- Implement evidence-based interventions to improve mobility, strength, balance, and overall function.
- Communicate effectively with patients, family members, and healthcare professionals to ensure seamless coordination of care.
- Educate patients and their caregivers on safe exercise techniques, proper body mechanics, and strategies for maintaining independence.
- Document patient progress, identify areas for improvement, and adjust treatment plans accordingly.
- Pennsylvania licensure as a Physical Therapist (PT)
- Minimum one year of recent experience in home health or similar setting
- Strong interpersonal skills, with ability to build rapport with diverse populations
- Excellent communication and documentation skills
- Ability to work independently, prioritize tasks, and manage time effectively
