Home Health Physical Therapist Opportunity

Amedisys, a leading provider of home health, hospice and high-acuity care, is seeking a skilled Physical Therapist to join our team. As a Physical Therapist with Amedisys, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans to help patients maintain, restore, and improve movement, activity, and function.
  • Provide interventions, conduct reexaminations, and modify interventions as necessary to ensure optimal patient outcomes.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.

Requirements:

  • Current license to practice physical therapy specific to the state you're assigned to work in.
  • Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
  • One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
