Nurse Care Manager Job Description

Avera@Home Yankton seeks a skilled Registered Nurse (RN) to serve as a Case Manager, delivering patient-centered care and coordinating services with multidisciplinary teams.

Salary Information

Requirements and Qualifications
  • Licensure: License as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure is required.
  • Education: Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
  • Experience: Minimum 2 years related experience and/or training; or equivalent combination of education and experience preferred.
