The Franklin County Board of Developmental Disabilities is committed to supporting individuals with developmental disabilities and their families through person-centered services and advocacy. We strive to create opportunities for individuals to live independently and participate fully in their communities.

Salary and Benefits:

\

$55,000 - $65,000 per year, based on experience. Tuition reimbursement, on-site fitness center, generous health care package, childcare assistance, 14% employer contribution towards retirement plan, deferred compensation options, paid holidays, and excellent leave policies.

Job Responsibilities:

\

As a Service Coordinator, you will provide person-centered services to individuals with developmental disabilities, including:

  • \
  • Identifying, referring, accessing, and linking eligible individuals with needed services\
  • Coordinating services among service providers and monitoring Individual Service Plans (ISPs)\
  • Writing and preparing reports, correspondence, case notes, assessments, and ISPs\
  • Maintaining current files, records, and other materials\
  • Attending and/or conducting meetings related to assigned caseload\
  • Consulting with service providers to improve or expand existing services\
  • Participating in community engagement activities to promote awareness of service coordination\

  • Bachelor's Degree in Human Services or related field\
  • Valid Ohio Driver's License\
  • Ability to travel to various parts of the county\
  • Strong communication and interpersonal skills\
  • Ability to work independently and as part of a team\
  • Knowledge of disability support services and community resources\
  • Mandated reporter certification\
  • Ability to maintain confidentiality and handle sensitive information\

Scope of Work:

The scope of work for this position includes approximately 60% administrative tasks and 40% practical experience. You will spend most of your time working on service plans, assessments, and case notes, as well as engaging in in-person meetings, home visits, and consultations with service providers.
