Skilled Nursing Care Professional

We are seeking a skilled nursing care professional to join our team as a Licensed Practical/Vocational Nurse (LPN) in home healthcare. As a novice nurse with less than 12 months of experience, you will be responsible for providing and documenting skilled nursing care in accordance with care developed for each individual patient.
Responsibilities- Utilize the nursing process to plan, implement, and evaluate patient/client care and/or contracted services
- Demonstrate on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders
- Identify and document changes in patient's both physiologic and psychosocial condition
- Notify Director/Supervisor if applicable on the following:
- Physician notification to obtain necessary orders for intervention(s) per state regulations
- Changes in patient/client condition
- Adhere to Maxim's policies and procedures and applicable protocols
- Identify and prioritize actions based upon patient/client care requirements
- Assure patient safety at all times and recipient of services provided
- Verify that documentation reflects patient/client collaboration, participation in plan of care, expected patient/client outcomes, and selected nursing interventions
- Perform interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies and patient/client care outcome standards as applicable.
- Monitor and document effectiveness of interventions, treatments, and medications
- Evaluate patient/client and provides a safe, comfortable and therapeutic patient/client environment
- Monitor the safety and effectiveness of the environment and equipment including universal precautions
- Participate in activities to facilitate good interpersonal communications
- Communicate observations and significant changes in patient/client status to the Director of Clinical Services and/or Clinical Supervisor /Case Manager or licensed designee
- Document all physician collaboration/coordination of care in the medical record and communicates this information as needed
- Attend case conferences, team meetings, education in-services as required
- Support professional practice by observing the following:
- Nurse Practice Act
- Standards of Nursing Practice
- Standards of Patient Care
- Standards of Accrediting Body (ACHC, Joint Commission, etc.)
- State and Local Laws
- Patient Bill of Rights
- Maxim Healthcare, Nursing, and Client's Hospital Policies
- Performs other duties as assigned
To be considered for this role, you must meet the following requirements:
- Graduate of an accredited school of nursing
- License Required
- 3.0 GPA preferred
- Complies with accepted professional standards and practice
- Current CPR
- PPD or chest X ray if applicable
- Successful completion of Maxim Homecare Novice Nurse Program orientation and training
- Proficiency in the English language required
- Excellent communication skills
- Technological proficiency, including mobile devices for documentation
The estimated annual salary for this position is $55,000-$65,000, depending on location and experience.
