About the Role

This Rehabilitation Coordinator position is an exciting opportunity to make a difference in the lives of severely mentally disabled adults. Located in the North Hollywood, CA area, our residential facility seeks a dedicated professional to provide various services under the direction of the Program Manager.

As a Rehabilitation Coordinator, you will play a vital role in ensuring the well-being and success of our clients. Your responsibilities will include scheduling, care and supervision, and providing emotional support to help clients achieve their personal goals.

Duties and Responsibilities
  • Monitor, care, and supervise mentally ill clients on-site, providing a safe and supportive environment.
  • Develop, implement, and document individualized client/resident needs and service plans, tailored to their unique requirements.
  • Implement schedules of client's/resident's programs and services at the facility, ensuring seamless delivery.
  • Observe and interact with clients, responding helpfully to their problems and needs, and providing crisis intervention when necessary.
  • Perform emergency procedures as required and assist with organizing and supervising group rehabilitation services daily.
  • Provide modeling and instruction for clients in daily living skills such as personal care, interpersonal relations, domestic skills, utilizing community resources, coping skills, and symptom management techniques.
  • Complete client progress notes in a timely manner daily and achieve individual monthly unit production goals per county contract.
  • Participate in staff meetings and training activities as required, maintaining confidentiality at all times.
Requirements and Qualifications
  • A minimum Bachelor's degree or two years of experience providing services to mentally ill or other disabled populations desired, or a combination of education and experience.
  • A valid California driver's license, reliable automobile, and good driving history, demonstrated by DMV record check.
  • Current certification in CPR and first aid is needed but not required.
  • Be in good health and physically, mentally, and occupationally capable of performing duties, verified by recent health screening and TB tests.
  • Good written and verbal communication skills, personal characteristics of warmth, enthusiasm, sense of humor, responsibility, and respect for others.
Compensation

The estimated salary range for this position is $45,000 - $65,000 annually, commensurate with experience.
