Job SummaryWe are seeking a highly skilled Registered Nurse (RN) to join our team at Colonial Oaks Health and Rehabilitation. As a Nursing Professional, you will play a vital role in providing high-quality patient care, collaborating with interdisciplinary teams, and promoting a culture of excellence within our organization.
Key Responsibilities- Provide direct patient care, including assessments, planning, implementation, and evaluation of resident health needs
- Develop and implement individualized care plans that meet the unique needs of each resident
- Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes
- Maintain accurate and up-to-date medical records, including documentation of resident progress and any changes to care plans
- Participate in quality improvement initiatives to enhance resident satisfaction and care delivery
- Current RN licensure in good standing, valid in the State of Indiana
- Graduate of an accredited nursing program with a minimum of one year of experience in long-term care or acute care settings
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations
- Basic computer skills, including proficiency in electronic medical records systems
- Competitive hourly rate: $40-$50 per hour, depending on experience
- $10,000 sign-on bonus for new hires
