**Salary Estimate:** $80,000 - $110,000 per year, based on experience and qualifications.
**Job Description:**
The RN, Care Manager will be responsible for evaluating members' needs and requirements to achieve and/or maintain an optimal wellness state. This role involves guiding members and their families toward interaction with resources appropriate for the care and well-being of members.
**Key Responsibilities:**
- Evaluate members' physical, environmental, and psychosocial health issues
- Develop and implement comprehensive care plans to address members' needs
- Collaborate with a multi-disciplinary team to manage members' health issues
- Travel to locations within the service area through a reliable means of transportation
**Requirements:**
- Graduate of an accredited program of professional nursing (ADN/BSN)
- Three years of experience in clinical nursing
- Current RN license in good standing with the State of Alabama Board of Nursing
- Valid driver's license in good standing
