Join us at MelroseWakefield Healthcare as a Medical Care Professional, where you will play a vital role in delivering exceptional care to our patients. In this Licensed Practical Nurse (LPN) position, you will work closely with our healthcare team to provide professional and non-professional nursing care services in accordance with physician orders.
About the JobThis is an exciting opportunity for a dedicated individual to join our team of medical professionals. As a Medical Care Professional, you will be responsible for implementing the medical/nursing plan of care, administering medications, and performing various procedures necessary for patient care under the direction of a Registered Nurse.
- You will provide direct patient care within the primary nursing model to ensure continuity of care for all patients within your caseload in a safe and efficient manner, maintaining patient rights and dignity at all times.
- You will perform technical aspects of patient care (medications, treatments, etc.) safely, according to established policies and procedures, and the Nurse Practice Act.
The ideal candidate will possess excellent communication skills, knowledge of the nursing process, and the ability to implement a nursing plan of care. You will also have the skills and knowledge necessary to provide care to patients throughout the lifespan, considering aging processes, human development stages, and cultural patterns.
Responsibilities- Assess and reassess pain, utilizing appropriate pain management techniques, and documenting pain management.
- Communicate all changes in patient condition immediately to the Registered Nurse.
- Bases all nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
- Establishes a positive relationship with patients and families, introducing yourself to patients and families as their nurse, providing teaching and counseling according to their needs and ability to understand.
- Reviews/educates discharged patients and families on ADLs, medications, treatments, and provides appropriate discharge instruction sheets.
- Documents all nursing interventions and patient responses in the medical record for all assigned patients.
In this role, you will have the opportunity to work collaboratively with peers and colleagues to create a positive work environment, while also participating in performance improvement activities to support and improve patient care.
Physical RequirementsThe physical demands of this role include prolonged standing/walking, lifting, positioning, pushing, and transferring patients and equipment. You will also be exposed to hazardous chemicals, sick patients, bodily substances, noise, and possible exposure to radiation, lasers, electric shock, etc.
Skills and Abilities- Ability to effectively communicate with patients, families, physicians, and healthcare teams.
- Knowledge and application of the nursing process to implement a nursing plan of care.
- Possesses and applies the skills and knowledge necessary to provide care to patients throughout the lifespan.
