At CenterWell Home Health, we're dedicated to providing personalized care to patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you'll play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.

Key Responsibilities:
  • Evaluate and develop individualized plans of care for patients with speech, language, and swallowing disorders.
  • Provide expert therapy services to patients in their homes, including assessment, treatment, and education.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes, and participate in quality improvement initiatives.
  • Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements:
  • Master's degree in Speech-Language Pathology or related field.
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
  • Minimum 6 months of experience as a speech therapist or speech-language pathologist.
  • Home health experience a plus.
  • Current and unrestricted license to practice in the state of employment.
  • Current CPR certification.
  • Excellent communication and organizational skills.
