Aveanna Healthcare is a leading provider of home health services to medically fragile children and adults. We are seeking a skilled Home Health Nurse Practitioner to join our team in providing high-quality care to patients in the comfort of their own homes.

**Job Summary**

We are looking for a compassionate and dedicated Home Health Nurse Practitioner to provide medical care and support to a teenager with high acuity needs. The successful candidate will have experience working with patients who require complex care and be able to work independently as part of a multi-disciplinary team.

**Responsibilities:**

  • Implement physician-prescribed plans of care, conduct patient assessments, and coordinate care with other healthcare professionals.
  • Administer prescribed medication, treatments, and therapies, and maintain accurate records of patient care.
  • Provide education and training to family members on patient care and promote healthy habits.
  • Work closely with physicians, Case Managers, and other healthcare professionals to ensure seamless care coordination.

**Requirements:**

  • Bachelor's degree in Nursing or related field, and current licensure as a Registered Nurse (RN) in the state where the clinician will practice.
  • Current certification in CPR from the American Red Cross or American Heart Association.
  • Six months prior hands-on nursing experience, preferably in pediatrics or critical care.
  • Reliable transportation and ability to travel to patient locations.
