LinkJobs is partnering with a local primary care clinic to find a skilled Home Visiting Nurse Practitioner.

Responsibilities:
As a Home Visiting Nurse Practitioner, you will manage the transitional and chronic care needs of homebound patients. Your responsibilities will include:

  • Conducting assessments and evaluations of patients' current conditions
  • Taking past medical histories and reviewing symptoms
  • Performing general physical examinations and reviewing medications
  • Screening for depression and substance abuse
  • Identifying diagnoses and recommending treatment measures
  • Communicating findings to the office or supervising physician
  • Educating patients on disease prevention, medication, and compliance

Requirements:

  • Current NP license in the state of Georgia with no sanctions or restrictions
  • Ability to speak Russian
  • At least 1 year of clinical experience
  • Clinical background in adult, family, or geriatric specialties
  • Experience working with Electronic Health Records (EHRs)
  • Effective communication and smartphone skills
  • Reliable transportation and ability to work independently
  • Home care or home visiting experience (preferred)
