As a Discharge Care Manager at UnitedHealth Group, you will be part of a dynamic team that delivers innovative care solutions to millions of people worldwide. We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Discharge Care Coordinator.

In this role, you will be responsible for implementing day-to-day telephonic case management interventions for identified high-risk members. This includes assessing and interpreting member needs, identifying solutions to help members live healthier lives, and collaborating with healthcare providers to optimize patient care.

Primary Responsibilities:

  • Evaluation of member discharge needs, including delays in care and readmission prevention planning
  • Collaboration with providers and members to coordinate care post-discharge
  • Participation in rounds with the Medical Director to discuss cases as needed
  • Identification of internal or community-based program support or resources
  • Coordination with facility Discharge Planners to ensure post-hospital services are arranged prior to discharge
  • Assistance with coordination of difficult cases requiring placement in an alternate level of care facility
  • Documentation of discharge activities, as outlined in standard operating procedures and data entry strategies
  • Participation in team meetings, education discussions, and related activities
  • Collaboration with team members in a matrix environment to ensure an end-to-end positive experience for members, providers, and care teams

We offer a competitive salary range of $58,300 to $114,300 annually, based on factors such as location, education, work experience, certifications, and more. Our benefits package includes a comprehensive benefits program, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility requirements).
