About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Supportive Care team at Southcoast Health System. As a key member of our team, you will provide high-quality patient care and support to individuals and families in need of palliative care services.

Responsibilities
  • Provide direct patient care and support under the supervision of the Director of Supportive Care or designee.
  • Communicate effectively with patients, families, and healthcare teams to promote goal achievement and exceptional customer service.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Participate in ongoing education and training to stay current with best practices in hospice and palliative care.
Requirements
  • Graduate of an NLN accredited school of Practical Nursing.
  • Current LPN licensure in both RI and the Commonwealth of Massachusetts.
  • One (1) year of recent hospital, medical/surgical, or other relevant nursing experience.
  • Experience in hospice and/or palliative care preferred.
  • Proficient computer skills, including experience with various software, database, and electronic records programs.
