At Upward Health, we are changing the face of primary care by delivering high-quality, patient-centered services directly to our patients' homes. We are seeking an experienced Family Nurse Practitioner to join our team and provide exceptional care to individuals with complex needs.

We are a technology-enabled medical group that specializes in primary care and behavioral health. Our innovative approach focuses on reducing barriers to care, improving outcomes, and enhancing the overall patient experience.

The ideal candidate will have a strong background in primary care, urgent and emergent care for complex patients, and experience working in inpatient and outpatient or home care settings. A multi-state Advanced Nursing License is required, and prior experience with motivational interviewing, vulnerable populations, and trauma-informed care is highly valued.

This is a full-time opportunity that requires after-hours and weekend availability via phone on a recurring basis. The successful candidate will be able to foster a collaborative working environment with all members of the interdisciplinary team and demonstrate excellent written, computer, and oral communication skills.

As a member of our team, you can expect a competitive salary range of $120,000 - $150,000 per year, depending on experience, as well as comprehensive benefits, professional growth opportunities, and the satisfaction of making a meaningful difference in the lives of our patients.
