Support Professional with Client Care Expertise

We are seeking a highly skilled and compassionate Support Professional to join our team at Hearts & Hands of Care. This role involves providing personalized care and support services to individuals in their homes or community settings.
Responsibilities:- Deliver high-quality client care, ensuring the safety and well-being of service recipients.
- Implement individualized Service Plans, incorporating family visions and strengths.
- Maintain accurate and comprehensive documentation, including daily notes, contact records, and progress updates.
- Promote a strengths-based perspective, empowering individuals to achieve desired outcomes.
- Collaborate with multidisciplinary teams to address client needs and develop effective interventions.
- Strong interpersonal skills and customer service focus.
- Demonstrated ability to maintain confidentiality and handle sensitive information.
- Experience with Therap database (preferred) and proficiency in documenting interventions and client responses.
- Ability to work independently and as part of a team, communicating effectively with colleagues and stakeholders.
- High school diploma or equivalent required; relevant certifications and training (e.g., Mandt) preferred.
- Competitive salary: $42,000 - $60,000 per annum.
