La Clinica de Familia is seeking a skilled Chronic Care Specialist to join our team. As a key member of our healthcare team, you will play a vital role in supporting patients with high-risk health conditions to navigate the healthcare system.

The ideal candidate will have experience in patient education, motivational interviewing, and self-management goal setting. You will work collaboratively with multidisciplinary care teams to ensure seamless transitions of care and provide resources to clinic staff.

Key responsibilities include:

  • Supporting patients with high-risk health conditions to navigate the healthcare system
  • Collaborating with multidisciplinary care teams to ensure seamless transitions of care
  • Providing resources to clinic staff to support chronic care management

Requirements include:

  • Licensed Practical/Vocational Nurse or other credentialed health professional
  • Current BLS certification
  • 2+ years of experience in a physician's office, clinical, or hospital setting
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD
