Nurse Practitioner - Primary Care Specialist
Job Summary:
We are seeking a highly skilled Nurse Practitioner to join our team at Greater Good Health. As a Primary Care Specialist, you will be responsible for delivering exceptional patient care and working closely with our care team to provide holistic, high-touch, coordinated care for a panel of senior patients.
Responsibilities:
• Build a strong culture and foster community among patients and team members
• Welcome and care for patients and team members, listening to their needs and ensuring they are supported throughout their visit
• Delegate tasks to Medical Assistants to support delivery of procedures, telephonic support, and patient instructions
• Foster a culture of engagement and demonstrate Greater Good Health core values
• Deliver exceptional clinical care to patients, including well, acute, and chronic condition management
• Provide full scope of primary care services, including taking medical histories, examining patients, making diagnoses, and prescribing medication as allowable by state guidelines
• Perform Annual Wellness Visits and Health Risk Assessments, requiring a holistic view of health and a focus on thoughtful, accurate, and specific clinical coding and documentation
• Manage patient panels and 'quarterback' patient care with patient's team to deliver high-touch, high-quality care
• Order diagnostic tests and interpret results, perform appropriate screenings, tests, and in-office procedures, and provide patients with education on preventive measures, lifestyle management, and disease management
• Make referrals to other providers, specialists, testing facilities, agencies, hospitals, or ancillary providers, and develop and manage relationships with specialty consultants
• Monitor patients' post-hospitalization and conduct post-discharge follow-up visits, assist with telephonic triage and outreach as needed, and provide after-hours coverage on a rotation with other Greater Good Health clinicians
• Perform prior authorizations and peer-to-peer reviews as needed
Requirements:
• Active FNP or AGNP license in the state of Montana (Required)
• CPR/BLS Certification (Required)
• Prescriptive Authority by the MT state board of Nursing or Medical Examiners and DEA License (Required)
• Practice in Primary Care and independent patient care 3-5+ years (Required)
• Valid Montana Driver's License (Required)
• Proficiency with Microsoft Word, Excel, and PowerPoint (Required)
• Adult Gerontology specialty board certification (Preferred)
• Value-based panel management and care coordination experience (Preferred)
• New practice implementation and growth experience (Preferred)
• Electronic Health Record (EHR) experience (Preferred)
• Experience delivering educational sessions (Preferred)
