We are seeking a dedicated Full-time Registered Nurse in Home Health Care to join our team at BJC HealthCare.

**Overview**

B JC HealthCare offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Our specialty home care programs include adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

We provide care to thousands of patients in both Missouri and Illinois, serving more than 25 counties. As one of the largest home care networks in the region and one of the largest in the country, we strive to deliver exceptional patient care.

About the Position

The Home Infusion Therapy Department works closely with physicians, patients and caregivers to enable patients to remain or return home while receiving intravenous medications. Our services include 24 hour on-call pharmacy and nursing services, comprehensive patient education materials, patient assessment and drug administration monitoring, and coordination of professional services including home care nursing and laboratory services.

This role evaluates clients and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.

**Responsibilities**

  • A sse ss patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
  • D evelops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care.
  • P romotes respect, equity and empathy in interactions with diverse and vulnerable populations through care delivery (e.g. support for emotional, spiritual, and cultural preferences of patient, family and/or caregivers).
  • I mplements care by integrating data from the interprofessional team and critical thinking in a safe and timely manner.
  • E valuates changes in patient's condition, informs and collaborates with family and/or caregivers, and communicates with interprofessional team as changes occur in plan of care, updates plan of care in EMR.
  • E valuates current nursing care to ensure evidence-based practice and quality patient outcomes.

  • Bachelor's Degree - Nursing/Home Health Experience

**Salary and Benefits**

The estimated annual salary for this position is $65,000-$85,000.
