About the Role
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care in the comfort of their own homes. As a Homecare Specialist, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities
* Verify plan of care with the RN prior to each visit
* Collaborate with physician and RN frequently to ensure patient needs are being met; notify them as soon as possible when patient condition changes
* Teach patients and caregivers self-care techniques
* Check vital signs
* Review all medications and perform a medication reconciliation at each visit
* Perform specialized procedures ordered by physician (e.g., foley catheter change)
* Prepare equipment and materials for treatments using aseptic technique
* Complete documentation timely and accurately
Requirements
* One year of clinical LPN experience
* Current LPN license specific to the state(s) you are assigned to work
* Current CPR certification
* Valid driver's license, reliable transportation, and liability insurance
