Physical Therapist Role: Patient Care Specialist

About the Job
We are seeking a highly motivated and compassionate Physical Therapist to join our team at JAG Physical Therapy.
As a Physical Therapist, you will play a crucial role in assessing patients' strengths and weaknesses to create personalized treatment plans aimed at improving their functional independence. You will work collaboratively with an interdisciplinary team to provide high-quality patient care while maintaining confidentiality and compliance with healthcare regulations.
The ideal candidate will have excellent clinical decision-making skills, strong communication abilities, and a passion for delivering exceptional patient outcomes.
Key Responsibilities:
- Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans, in accordance with departmental guidelines and regulatory requirements.
- Develop and implement treatment plans based on evaluation results, goals, and best practices, collaborating with interdisciplinary teams and patients as needed.
- Provide education and training to patients and their support systems on ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration.
- Conduct musculoskeletal interventions, including therapeutic exercise, joint mobilization, and modalities, to promote patient recovery and functional independence.
- Oversee support staff, including PTA, ATC, and PT aides, to ensure seamless patient care.
- Promote JAG Physical Therapy's services through lectures and presentations in the community.
Requirements:
- Bachelor's or Master's degree in Physical Therapy; Doctorate degree preferred.
- Licensed Physical Therapist in the state of practice.
- Completed state-regulated CEU requirements.
- Valid CPR license.
- Experience with EMR software preferred.
- Excellent oral and written communication skills.
- Strong clinical decision-making skills.
- Ability to work independently with minimal supervision.
- Maintain a positive, professional demeanor at all times.
