Job Summary:

Advocate Aurora Health is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping patients achieve their goals and improve their quality of life.

Key Responsibilities:

  • Interpret physician referrals and evaluate patients' physical, perceptual, cognitive, social, and vocational abilities to determine their impact on daily living activities.
  • Develop and implement individualized treatment plans based on evaluation results, functional goals, and appropriate techniques.
  • Provide and/or fabricate assistive devices and/or splints to enhance patients' functional independence.
  • Monitor and evaluate treatment outcomes, adjusting individualized treatment plans as needed, and provide status updates to physicians.
  • Identify factors affecting patients' physical and psychosocial comfort and address these needs.
  • Plan for and complete patient discharge, including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
  • Complete all required documentation, including patient progress notes, team conference reports, discharge summaries, and records of patient care and progress.

Requirements:

  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) within 6 months.
  • Occupational Therapist license issued by the state of Wisconsin.
  • Bachelor's Degree in Occupational Therapy.
  • Intermediate computer skills.
  • Excellent communication and interpersonal skills.
  • Demonstrated organization, decision-making, and problem-solving skills.

Working Conditions:

  • Must be able to sit, stand, walk, lift, and squat throughout the workday.
  • Must be able to lift up to 50 lbs. from floor to waist, lift up to 10 lbs. from waist to overhead, and lift and carry up to 40 lbs. at waist height a reasonable distance.
  • Must be able to push/pull with 30 lbs. of force, perform a sliding transfer of 150 lbs. with a second person present, and position requires repetitive use of hands.
