Role Overview

We are seeking a skilled Patient Care Coordinator to join our team at Mass General Brigham in Newton, Massachusetts. This full-time role will be responsible for facilitating discharge plans and coordinating post-acute care services for patients admitted to various departments.

About the Role

The ideal candidate will have a strong background in nursing with experience in case management, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery. A current RN license in the Commonwealth of Massachusetts and a minimum of three years' clinical experience are required.

This position involves autonomy in decision-making, effective communication, and collaboration with healthcare providers, insurers, and community organizations. The successful candidate will have excellent organizational skills, basic computer proficiency, and knowledge of Microsoft products.

Key Responsibilities
  • Facilitate discharge plans and coordinate post-acute care services for patients
  • Collaborate with healthcare teams to ensure continued appropriate levels of care
  • Review patients' health records and communicate with healthcare teams regarding medical necessity and level of care
  • Participate in quality and risk management case finding activities
  • Serve as a resource for physicians, hospital staff, and patients/families on insurance coverage, community resources, and post-acute care options
Requirements
  • Graduate of an accredited school of nursing; BSN preferred
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts, with a minimum of three years' clinical experience
  • Evidence of continued education and professional development; CCM or ACMA certification preferred
  • Prior experience in general acute care hospital experience, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery
  • Basic computer skills, including knowledge of Microsoft products and efficient data entry ability
