About the Role:

We are seeking a skilled Patient Care Therapist to join our team in Senior Living at LHC Group. As a trusted member of our care team, you will have the opportunity to build meaningful connections with patients and their families while delivering high-quality, patient-centered care.


Key Responsibilities:

Evaluating patients to determine rehab potential, identifying needs, establishing individualized treatment plans, and setting goals for optimal care outcomes is crucial. You will also set initial short-term goals (STGs) and long-term goals (LTGs), maintain accurate equipment logs, and follow medical regimens developed for each patient.


Requirements:

To be successful as a Patient Care Therapist, you must possess current licensure as a Physical Therapist in your state of practice. Additionally, you should hold current CPR certification and have a valid driver's license, vehicle insurance, and access to reliable transportation or public transportation.


Salary Information:

The estimated salary range for this position is $85,000 - $110,000 per year, based on industry standards and national averages. This figure may vary depending on location, experience, and qualifications.
