Care Manager for Senior Living Facilities

Job Summary
We are seeking an experienced LPN/LVN or state/province specific licensed nurse credential holder to join our team as a Medication Care Manager at Sunrise of Naperville North. This is a challenging yet rewarding role that requires strong leadership and communication skills, as well as a passion for delivering high-quality care.
Responsibilities
- Administer medication and treatments in a safe and organized manner
- Conduct Service and Health Updates as directed by the Resident Care Director
- Participate in the development of Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates
- Transcribe orders
- Partner with community team to ensure compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements
- Report all accidents/incidents immediately
- Report all unsafe and hazardous conditions/equipment immediately
- Maintain confidentiality of all resident information
- Assess residents to determine need for "as needed medication"
- Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift
Qualifications
- LPN, LVN, or state/province specific licensed nurse credential
- In states/provinces where appropriate, must maintain certifications
- CPR and First Aid certification
- Must be at least 18 years of age
- Previous experience working with seniors preferred
- Desire to serve and care for seniors
- Ability to make choices and decisions and act in the resident's best interest
- Ability to react and remain calm in difficult situations
- Ability to handle multiple priorities
- Possess written and verbal skills for effective communication and level of understanding

Compensation
The estimated salary for this position is $ $74,000 per year, based on the title, job description, and location. This figure may vary depending on individual qualifications and experience.
