Key Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is tasked with assessing and evaluating patient care needs concerning functional mobility, endurance, pain management, and other medical conditions. Based on these evaluations, the Physical Therapist collaborates to formulate a treatment plan, implements interventions aimed at enhancing the patient's well-being, and monitors the patient's progress.

  • Assess the patient's functional capabilities and physical therapy requirements, assisting the physician in crafting the therapy care plan.
  • Monitor and report the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
  • Provide treatment to alleviate pain, restore function, and maintain optimal functioning according to established standards of practice.
  • Educate and inform members of the healthcare team, the patient, and their family/caregivers about the care plan, functional limitations, and progress towards goals.

Required Qualifications

Licensure Requirements

  • Valid Physical Therapy license in the state of practice.
  • Current CPR certification.
  • A valid driver's license, vehicle insurance, and access to a reliable vehicle.
