MDS Coordinator

We are seeking a highly skilled and experienced MDS Coordinator to join our team at Nursecare of Buckhead. As a key member of our clinical department, you will play a vital role in ensuring the timely and accurate completion of the RAI and Care Management process.
Key Responsibilities
• Coordinate and facilitate the RAI and Care Management process from admission to discharge, ensuring compliance with company policies, state and federal guidelines, and other relevant regulations.
• Collaborate with the Interdisciplinary Team to assess resident needs and develop effective care plans.
• Work closely with Regional Clinical Reimbursement Consultants to ensure accurate and timely resident assessments.
• Coordinate daily care management meetings, review resident care, and set Assessment Reference dates.
• Designate responsibility for completing sections of the assessment to the Interdisciplinary Team.
• Complete accurate coding of the Minimum Data Set (MDS) with information obtained from medical record reviews, observations, and interviews with facility staff, residents, and family members.
• Maintain tracking systems for MDS schedules, including time frames and due dates.
• Enter resident information into computerized MDS programs, ensuring accurate and frequent data entry.
• Coordinate care plan conferences with the Interdisciplinary Team, residents, and families.
• Obtain, review, and maintain state and federal reports, making timely corrections as needed.
• Coordinate education related to the RAI/Care Management process for facility staff.
• Submit required documentation electronically to the state database and other entities, adhering to company policy and state and federal regulations.
• Complete required forms and documents in accordance with company policy and state and/or federal regulations.
• Attend Interdisciplinary Team meetings and other meetings to gather information, communicate changes, and maintain and update records.
• Participate in quality assurance activities and continually update knowledge related to data entry and computer technology.
Requirements
• RAC Certified
• RN required
• Must be in good standing with the Board of Nursing
• Knowledge of Medicare process preferred
• Current state nurse license
• CEUs as required by the state
Working Conditions
This position requires working in a normal office environment, with occasional lifting of medium to heavy objects, including assisting with the movement of residents. The employee may be exposed to blood or other bodily fluids, fumes, or airborne particles, and toxic or caustic chemicals.
