We are seeking a skilled Licensed Practical Nurse to join our team at Catholic Charities Family Community Services. As a key member of our healthcare team, you will provide high-quality patient care and support in a residential setting.
Key Responsibilities- Provide medication management and administration to clients in accordance with authorized prescriptions
- Document medication administration and track client compliance with required medications
- Collaborate with other healthcare professionals to ensure comprehensive client care
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination
- Valid Licensed Practical Nurse (LPN) license
- Minimum of 2-3 years of experience in nursing services, preferably in a residential setting
- Ability to work collaboratively as part of a healthcare team
- Excellent communication and interpersonal skills
