Your Key Responsibilities:

  • Assess and implement care strategies tailored to individual patient needs, ensuring the most appropriate level of care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage care plans throughout the continuum of care, serving as a single point of contact.
  • Communicate effectively with stakeholders to ensure quality coordinated care and services are provided efficiently.
  • Advocate for patients and families as needed, ensuring their needs and choices are represented and supported by the healthcare team.

Requirements:

  • A RN in the state of Texas, Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Masters Social Worker (LMSW).
  • 2+ years of experience working within the behavioral health setting.
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook.
  • Reliable transportation and the ability to travel up to 75% of the time in the assigned region.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).

Preferred Qualifications:

  • Medicaid Waiver experience.
  • Case Management and/or Care Coordination experience.
  • Behavioral health experience.
  • Field-based work experience.
  • Experience with electronic charting.
  • Background in managing populations with complex medical or behavioral needs.
  • Knowledge of community resources.
