Hospice Care Specialist

As a Hospice Aide at Virtua Health, you will provide compassionate and supportive care to patients with terminal illnesses, improving their quality of life under the supervision of a registered nurse.
Responsibilities include personal care services, household duties, and documentation of care provided.
Relay pertinent information to the assigned hospice registered nurse and participate in performance improvement activities.
Reinforce education and provide emotional support to patients and families regarding individual care needs, wellness, and safety issues.
Practice standard precautions and patient safety principles.
Identify and communicate changes in patients' status to RN and respond appropriately.
Participate in interdisciplinary group meetings and provide meaningful feedback as requested.
Maintain departmental level competencies.
Requirements:
- Knowledgeable of and supports the plan of care under the direction of the RN.
- Provides compassionate and meaningful personal care and household chores to/for the patient as established in the plan of care.
- Submits all documentation (care plan, time sheets, API, etc.) accurately and timely.
- Identifies and communicates changes in the patients' status to RN and responds appropriately.
- Reinforces education and provides emotional support to the patient and family / responsible party regarding individual care needs, wellness, and safety issues.
- Practice standard precautions and patient safety principles.
- Identifies and partakes in performance improvement activities.
Preferred Qualifications:
- Nurse Aide experience preferred.
- High School Diploma or equivalency preferred.
- Successful completion of State Certified Aide course.
Training / Certification / Licensure:
- License as a Certified Home Health Aide and current standing on the State Nurse Aide Registry.
- Must have current proof of valid driver's license and auto insurance.
- Current Certification CPR and BLS.
