Position Overview:

The Mental Health Specialist will conduct brief screenings to assess the mental health and support needs of patients, perform comprehensive bio-psychosocial assessments, and provide early interventions, referrals to specialty behavioral health services, and collaborate on integrated treatment plans. The ideal candidate will have a strong background in patient education and/or chronic disease management, effective communication skills, and the ability to work with clients through telehealth and in person.

  • Coordinate care
  • Crisis intervention and community collaboration

Requirements:

  • Licensed Mental Health Practitioner (LMSW, LCSW, LMHC, PhD, PsyD, LMFT)
  • Experience in or a desire to learn about patient education and/or chronic disease management
  • Ability to effectively engage clients in a therapeutic relationship
  • Ability to work with clients through telehealth and in person
  • Experience with screening and assessment for common mental health/substance use disorders
  • Comfort with the pace of primary care and working with an interdisciplinary team
  • Strong communication skills

Preferred Qualifications:

  • Yiddish language competency
