The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician's orders when applicable.


Key Responsibilities:
  • Performs initial comprehensive assessments and reassessments to evaluate patient rehabilitation needs, collaborating with the physician and healthcare team to modify the plan of care as necessary.
  • Develops and implements physical therapy plans of care in collaboration with the physician and healthcare team.
  • Assesses and evaluates patient therapeutic, rehabilitative, and functional status, as well as home environment, as applicable.
  • Provides physical therapy treatment and instructs patients, families, and caregivers in the use and care of therapeutic appliances.
  • Reports patients' reactions to treatment or changes in condition to the physician and/or designee.
  • Initiates physical therapy programs and instructs, consults, and advises other personnel and/or family/caregivers regarding certain phases of physical therapy.
  • Trains patients in the use of prosthetic devices and assistive devices, ensuring equipment used in patient treatment is in good working order.
  • Uses appropriate equipment to meet patient goals, cleans and maintains equipment according to manufacturer guidelines.
  • Prepares and submits clinical and progress summaries based on the attainment and progress of goals, and responses to treatment or changes in condition.
  • Participates in discharge planning and peer consultation processes.
  • Supervises Physical Therapy Assistants according to company policies/procedures and state regulations.
  • Provides/submits accurate and timely documentation in accordance with the plan of care and company policy and procedures.
  • Complies with applicable local, state, and federal laws and regulations regarding professional standards and practice standards.

Requirements:
  • Current licensure as a Physical Therapist in the state of practice.
  • Graduate of an accredited school of Physical Therapy.
  • One (1) year of prior professional Physical Therapy experience preferred.
  • Complies with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program when applicable.
  • Current CPR if applicable.
  • TB Questionnaire, PPD or chest x-ray if applicable.
  • Current Health certificate (per contract or state regulation).
  • Must meet all federal, state and local requirements.
  • Successful completion of new hire training as applicable to job site.
  • Understand patient confidentiality and HIPAA requirements.
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
  • Computer proficiency required.
  • Must be at least 18 years of age.
