We are seeking a skilled Child Welfare Specialist to join our team at Health Connect America, Inc.

About the Role

This is a critical position in our Foster Care program, responsible for overseeing the service planning, counseling, supervision, and case management of children and families within the foster care system.

Key Responsibilities
  • Develop and implement individualized treatment plans for children in foster care, taking into account their unique needs and circumstances.
  • Maintain detailed case records and complete weekly activities/status reports, ensuring accurate and up-to-date information.
  • Coordinate and link children to appropriate services, minimizing care fragmentation and ensuring continuous access to necessary services.
  • Foster positive relationships between birth parents, foster families, and children, promoting family success and child welfare.
  • Monitor and continually evaluate each child's care plan, adjusting strategies as needed to meet evolving needs.
  • Recruit, assess, and train foster parents, ensuring they have the necessary skills and support to provide a safe and nurturing environment.
Requirements
  • Bachelor's degree in social work or a related field, such as sociology, psychology, special education, or counseling.
  • At least one year of experience in providing casework services to children and families.
