Job Description:

A Diabetes Educator plays a vital role in helping patients manage their diabetes effectively. As a key member of our Value Based Medicine team, you will be responsible for educating patients on how to maintain a healthy lifestyle, monitor blood glucose levels, and adhere to medication regimens.

Responsibilities:

  • Teaching patients about diabetes management strategies
  • Developing personalized plans to improve patients' quality of life
  • Collaborating with healthcare providers to ensure seamless care coordination
  • Educating patients on safe medication use and proper equipment handling
  • Maintaining accurate records of patient progress and outcomes
  • Supporting Value Based Medicine initiatives to optimize patient outcomes

Requirements:

To succeed in this role, you will need:

  • A High school diploma or equivalent degree
  • Certification as a Certified Diabetes Educator (CDE)
  • 2+ years of experience working with patients with diabetes
  • 1,000 hours of diabetes self-management education
  • A minimum of 15 hours of continuing education coursework annually related to diabetes treatment and care

Physical Requirements:

This role requires:

  • Regular walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling
  • The ability to lift and support the weight of 35 pounds
  • Excellent concentration skills
  • Prolonged computer use
