Overview:
We are seeking a dedicated and compassionate Registered Nurse to join our team at Yale-New Haven Health. As a key member of our healthcare team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the Role:
This is a dynamic and challenging role that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care. As a Registered Nurse on our team, you will be responsible for assessing, planning, implementing, and evaluating individual patient care according to unit policies and procedures.
Key Responsibilities:
- Assess patients' physical and psychosocial conditions on an ongoing basis throughout the continuum of care.
- Develop, communicate, and document plans of care reflecting individualized patient problems and interventions to achieve desired outcomes.
- Implement and coordinate plans of care in collaboration with the healthcare team.
- Evaluate the effectiveness of plans of care, patients' responses to care, and readiness for discharge. Revise plans of care as needed and make referrals where indicated.
- Plan for education after assessing individual learning needs and readiness of patients and families. Provide educational opportunities and document outcomes.
- Function as a preceptor to new staff.
- Function as a charge nurse.
- Demonstrate ability to provide a safe environment for self and patients.
- Meet performance expectations for Customer Service, Teamwork, Resource Utilization, and Staff and Self Development as outlined in performance reviews.
- Perform other duties as assigned or directed to ensure smooth operation of the department/unit.
Requirements:
To be successful in this role, you will need:
- A valid State of Connecticut Registered Nurse license.
- BLS certification required.
- ACLS certification required within 6 months.
- Graduate of an accredited school of nursing; BSN Preferred.
- Proficient in use of computer software to include electronic patient records.
- Ability to use internet for literature searches regarding care for patients.
Salary:
The estimated salary for this position is between $80,000 - $120,000 per year, depending on experience and qualifications.
