Job Summary: We are seeking a highly skilled Advanced Care Professional to join our nursing team. As a member of our interdisciplinary healthcare team, you will work under the general supervision of experienced clinicians and department physicians to deliver high-quality patient care.

About the Role: This full-time position is responsible for providing direct patient care and collaborating with colleagues to develop and implement individualized care plans. The ideal candidate will possess excellent assessment, communication, and interpersonal skills, as well as experience working in a fast-paced environment.

Responsibilities:

  • Apply the nursing process to assess, plan, implement, and evaluate patient care
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions
  • Communicate effectively with patients, families, and colleagues to promote a culture of safety and respect
  • Participate in ongoing education and professional development to stay current with best practices
  • Maintain accurate records and documentation to support quality improvement initiatives

Requirements:

  • CURRENT RN licensure in the State of New Hampshire
  • MINIMUM 1 year of nursing experience in an acute care setting
  • BCLS certification required
  • Excellent assessment, communication, and interpersonal skills
  • Able to work well in a fast-paced environment and respond calmly during crisis situations
  • Computer skills required
