About the Role

We're seeking a highly skilled and compassionate RN or LPN to join our team as a Clinical Care Manager - Rehabilitation Services. In this role, you'll be responsible for coordinating and directing the care and services provided by our skilled nursing community to ensure the best possible outcomes for our residents.

You'll work closely with our interdisciplinary team to develop and implement effective care plans that address the unique needs and goals of each resident. Your expertise and leadership will be invaluable in promoting a culture of quality and excellence within our organization.

Key Responsibilities
  • Coordinate and direct the care and services provided by our skilled nursing community
  • Develop and implement effective care plans that address the unique needs and goals of each resident
  • Work closely with our interdisciplinary team to ensure seamless transitions and continuity of care
  • Provide coaching and training to staff members to enhance their skills and knowledge
  • Maintain accurate records and reports related to resident care and services
Requirements
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing
  • Bachelor of Science in Nursing (BSN) preferred
  • Minimum 2 years of related nursing experience, preferably in senior services
  • Minimum 3 years of supervisory experience required
  • Knowledge of state and federal laws and regulations for skilled nursing facility operation
