We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at East Boston Neighborhood Health Center. The successful candidate will have the opportunity to work in a dynamic community health setting, providing high-quality patient care and collaborating with a multidisciplinary team.

About the Role

This LPN position is a full-time role that requires a strong commitment to delivering exceptional patient outcomes. As an LPN at East Boston Neighborhood Health Center, you will be responsible for assessing patients' health status, developing individualized care plans, and implementing evidence-based interventions. You will also contribute to the overall success of our healthcare team by participating in interdisciplinary collaborations and sharing best practices.

Key Responsibilities
  • Patient Assessment: Conduct comprehensive assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Care Planning: Develop and implement individualized care plans that incorporate the latest clinical research and best practices, taking into account patients' unique needs and circumstances.
  • Nursing Interventions: Implement nursing interventions that promote optimal patient outcomes, including medication management, wound care, and education on healthy behaviors.
Requirements and Qualifications

To be considered for this exciting opportunity, you must possess a current LPN license in Massachusetts and 1 year of relevant experience in a community health or ambulatory care setting. Strong communication and interpersonal skills are essential, as well as the ability to work effectively in a fast-paced environment.
